Brief bio or Site description

Our ambulatory care pharmacy team is dedicated to reducing hospital readmissions through proactive medication
management. We collaborate closely to ensure patients are discharged with the appropriate medications and conduct
thorough post-discharge follow-up calls. During these calls, we confirm medication education, verify the accuracy of the
medication list, and address any potential barriers to accessing prescribed treatments.

What is your or your sites philosophy on educating students and/or providing pharmacy services?
Our philosophy on educating students and providing pharmacy services is centered on fostering both clinical expertise
and compassionate patient care. We emphasize the importance of staying current with the latest developments in
disease states and treatment options, while also encouraging a patient-centered approach. Empathy is a key value we
instill, ensuring that our students and staff understand the importance of treating each patient as an individual,
addressing both their medical and emotional needs.


What is a fun fact about you or the site?
I have two miniature dachshunds
