It is with great pleasure that I highlight the exceptional performance of Fabian. Throughout this rotation, Fabian has demonstrated remarkable dedication and expertise across multiple projects. Notably, he has created comprehensive risk assessments for hazardous drugs in compliance with USP 800 guidelines, ensuring safety and regulatory adherence. Fabian has effectively addressed complex drug information queries, including the oral dose regimen for vancomycin for C. difficile prophylaxis in non-transplant patients, and identified which classes of medications should be avoided in treating hypertension in patients with cocaine toxicity, providing thorough explanations for each. His ability to determine if continuous use of IV fentanyl leads to urinary retention showcases his deep understanding of pharmacological effects and patient care. In addition to his clinical ability, Fabian redesigned and formatted a TPN order template, significantly enhancing TPN order documentation and clinical note efficiency. He also played a pivotal role in developing and delivering a PowerPoint presentation and weekly in-service sessions to our pharmacy staff on the safe handling of hazardous drugs, contributing to improved staff knowledge and safety practices. Fabian’s contributions extend to updating the sterile compounding beyond-use date reference chart with the latest USP 797 guidelines, conducting operational audits on high-expense medications, and performing weekly vancomycin-kinetic dosing and clinical interventions during interdisciplinary rounds. His participation in weekly calculation questions, including alligation calculations, pediatric medication dosing, and Immune globulin IVIG dose infusion rate calculation, further defining his comprehensive skill set. Moreover, Fabian has diligently observed and documented glove fingertip testing results and media-filled test results post-incubation per USP 797 guidelines, ensuring the highest standards of sterile compounding practices. His collaboration on an in-service to the critical care team regarding blood pH effects during targeted temperature management of post-cardiac arrest patients, explaining pCO2 changes and respiration rate management strategies, was invaluable. Fabian’s final PowerPoint patient case presentation titled “Amphotericin B for the Treatment of Cryptococcal Meningitis in HIV-Positive Individuals” exemplifies his ability to synthesize and present complex clinical information effectively. Fabian has consistently taken on multiple tasks, exceeding expectations at every turn. His inquisitive nature, exemplary clinical interventions, and advanced capabilities as a pharmacy student set him apart as a standout performer. I am confident that Fabian will continue to excel in his future endeavors and make significant contributions to the field of pharmacy.
What was your favorite part of this rotation?
My favorite aspect of this rotation was the diverse range of experiences my preceptor encouraged me to engage in. Since I have a strong interest in infectious diseases, I had the chance to assist with drug-information inquiries that required literature reviews on ID-related topics. I also joined internal medicine and ICU rounds, where I contributed intervention recommendations under my preceptor’s supervision. Additionally, I had the unique opportunity to create and present a patient case on cryptococcal meningitis at the conclusion of my rotation—an experience uncommon for third-year students. Overall, I greatly valued learning about the healthcare team’s dynamics during rounds and contributing to interventions aimed at enhancing patient safety and outcomes.
What advice do you have for pharmacy students preparing for rotations?
Do not be afraid to ask questions. If you are unsure about instructions or are curious to learn something, asking questions is one of the most genuine ways to show respect. It demonstrates humility by acknowledging what you don’t know while also recognizing the expertise of the person you’re asking. Additionally, be proactive in seeking opportunities by requesting projects or assignments that you find interesting or engaging. Taking the initiative shows your enthusiasm and curiosity, which will not go unnoticed by your preceptor.
What are your plans after graduation?
Upon graduation, I plan to pursue a residency in South Florida with the goal of securing a Clinical Pharmacist position within a health system. My current interests are in infectious disease and ambulatory care, and I am excited for my APPEs to either solidify these interests or expose me to new areas of pharmacy.
